In your case some leaking petrol has probably flushed it out. Normal in fact.

The cooling comes out through the exhaust as an exit point and for cooling the exhaust. I did change the muffler filter during the service installed with a new rubber gasket wondered if I should have used a sealant of the gasket. The water pumps really well no issue there. Yep its supposed to be comming out of there.

If your tell-tale was weak impeller worn and weak then you would have less water going through the by-pass to the exhaust. Yes this sounds like the residue of the fuel oil mix if the motor is running on all cylinders it can be a result of a rich running condition in the carburetors or the result of having to use the choke more than usual if the motor exhibits a rough running characteristic at idle or if it seems to use more fuel than before you could have a problem with one of the carburetors todays gas has additives that will wash the old deposits in the exhaust out. Water coming out of those 4 holes is good I would think indicating that water is coming through the engine and into the exhaust section most of which will go through the normal exhaust run. Id run it for 10 to 20 mins at around 14 revs and do the finger on the engine test you may not have a problem at all apart from a blocked tell tale.
Update to my problem. I tore into the head and exhaust cover all clear thermostat had some sand and grit in it cleaned it out. Checked the water pump again no problems there.
The engine is a thru hub exhaust and i noticed the water is poring out of the hub when it is running its like the water isnt even making it to the head. The two exhaust holes on the back of the mid section have water coming out. Outboard engines will discharge exhaust gases underwater typically through the propeller while the exhaust from inboard engines appears just above the waterline along with the discharge of cooling water.
